FMEA or Failure Mode and effect analysis is a risk management tool and can be brought to use at several places in a Lean Six Sigma initiative. In the pre-project stage can be made and made part of the Charter document on the risks on the project. In the measure or analyze some people build FMEA of the process map to help identify the contributing factors (or potential Factors). Improve has to have the FMEA as one must study how their improvement plan may go wrong and help bridge the gap. Control Phase cannot be made without a good FMEA document.
